1. What to do when a family member reveals information about your patient, but asks you to keep it confidential and not share it with the patient?
2. How much information should a patient know about what’s in his/her chart?
3. What happens to sensitive material in the chart?
4. What happens to the chart after a patient dies?
5. Who has access to patient information?
6. Legislative requirements around personal health information
