Educational Goal & Objective:

• Learn about the positive results from a community based falls prevention program (FIT) for seniors in the community
• Learn about the application of the population health approach to sustaining the FIT Program in the community through collaboration with community service providers
• Learn about the principles and strategies utilized to support a cross-sectoral partnership
