Cholinesterase inhibitors and Alzheimer's disease: A two-year study in the Sunnybrook dementia cohort
ANNALS OF NEUROLOGY
To assess cognitive and functional effects of cholinesterase inhibitors over two years, patients (Untreated34, Treated 58) meeting NINCDS-ADRDA criteria for probable AD at a university memory clinic underwent standardized assessments at baseline, one and two years F/U (NSD in F/U). Baseline assessment was within two months of start of treatment. Both groups received a careful evaluation of co morbid illnesses, concomitant medications, and vascular risk factors. There were no significant differences in detailed medical evaluations. Separate repeated measures MANOVAs (Bonferroni corrected) on the MMSE, Mattis Dementia Rating Scale (DRS) total score, its 5 subscores, tests of memory, language, visuospatial and executive functioning, and ADLs revealed significantly less decline in treated patients in MMSE, total DRS and conceptualization subscore, Boston naming and CVLT acquisition score. Less decline was seen in instrumental and basic ADLs and overall scores of initiation and planning but not performance. Durable effects were seen not only in memory and language, but also globally and particularly in the executive aspects of daily functioning with moderate to large effect sizes. Functional benefits, likely to be considered clinically relevant, occurred in parallel with cognitive benefits. Drs.
